Ashton Manual Benzodiazepine Tapers

If you have been on a benzodiazepine for years, especially at higher doses, tapering can feel impossible. Keystone Psychiatry offers structured benzodiazepine tapering using Ashton manual principles (one of only four in the state of Pennsylvania), built around slow, symptom guided dose reductions and close follow up.

This is not a one size fits all plan. The approach is individualized based on your medication, dose, duration of use, medical history, prior taper attempts, and current stress load. The goal is to reduce risk, improve tolerability, and help you stay functional while steadily working toward discontinuation.

Keystone Psychiatry has helped many patients come off long term benzodiazepines, including those on high doses and those who have struggled with past tapers. I am one of a small number of providers in Pennsylvania offering Ashton manual style tapers, with a focus on careful pacing, clear written plans, and consistent monitoring.

What is the Ashton Manual

The Ashton approach is a well known framework for benzodiazepine discontinuation that emphasizes gradual tapering, careful symptom monitoring, and individualized pacing. In some cases, it includes transitioning from a shorter acting benzodiazepine to a longer acting option to smooth interdose withdrawal, when clinically appropriate.

What to expect

This is not a fast taper as you would see at many other offices or in detox facilities. We start with a detailed review of your benzodiazepine history, including your current dose and timing, how long you have been taking it, other medications, prior taper attempts, and any withdrawal complications. If your dosing is inconsistent, the first step is usually stabilization on a reliable baseline. From there, we create a written taper plan with small, planned reductions and built in flexibility to hold or slow down based on symptoms every 4 weeks. Follow ups are scheduled regularly to review progress, adjust pacing, and address sleep disruption, rebound anxiety, and other withdrawal related symptoms.

Who this is for

This program is designed for people with long term benzodiazepine use, high dose regimens, tolerance, interdose withdrawal, rebound insomnia, or prior failed tapers due to symptom intensity or overly fast reductions.

Getting started

To begin, have your current medication list, your typical daily dosing schedule, how long you have been taking benzodiazepines, and any history of complicated withdrawal or seizures. If you have pharmacy fill history or records from prior prescribers, bring those as well.
